With your Grade 5 Get Active! or Grade 6 Stay Active! Card, you can drop in to a ton of sports and general admission programs … for free!

The Grade 5 Get Active! & Grade 6 Stay Active! Card program will be available to students registered in the Grade 5 or 6 curriculum, ages 9 - 12 years old, who are residents of Coquitlam, Port Coquitlam, Port Moody, Belcarra, or Anmore. Students in School District #43, independent and private schools, and home school are welcome to participate.

To receive the Grade 5 Get Active! or the Grade 6 Stay Active! Cards, students must visit a local community recreation facility at any time during September 8, 2009 - August 31, 2010.

Grade 5 GET ACTIVE Card
In a partnership to promote youth involvement in physical activity, Port Coquitlam, Coquitlam, and Port Moody are offering a Get Active! Card for all Grade 5 students giving them unlimited free admission to the drop-in swimming, skating and gymnasium-based activities in the Tri-Cities. The program is available to Grade 5 students for the period of September 8, 2009 - August 31, 2010. By offering free admission to drop-in activities, it is hoped this added incentive would attract more young people to recreational activities to support this movement towards active living.

Grade 6 STAY ACTIVE Card
The Grade 5 Get Active! Card has been extended to include Grade 6 students for the 2009/2010 school year.  This program is a transition card encouraging Grade 6 students’ continued participation in physical activity. This card will allow 12 free visits for all Grade 6 students, enabling them to continue to develop healthy lifestyles at this age.  Students will be able to use their 12-visit card within the period of September 8, 2009 – August 31, 2010.

Registration
To receive and activate their membership card, students must bring the following documentation with them to the registration desk:

• a letter of verification from School District #43 identifying them as a Grade 5  or 6 student
• birth certificate or Care Card showing the student is of the applicable age to enter either Grade 5 or Grade 6 as of December 31, 2009
• proof of residency – Port Coquitlam, Coquitlam, Port Moody, Belcarra, or Anmore address

It is the student’s responsibility to keep the card safe. If the card is lost or damaged, replacement cards are available at a nominal fee of $5.

*Free admission to drop-in programs and public admissions is valid only for the period of the Program, with either a Grade 5 Get Active! or Grade 6 Stay Active! Card.
